We also discussed how vitally important it is for people with sickle cell disease to have a primary care provider and the reason and why. We also discussed the importance of the patient and provider developing a care plan. We briefly discussed transitioning from pediatric to adult practice and ways that patients can keep track of appointments; MyChart is a helpful tracking tool. Finally, we also discussed clinical navigators role in assisting with care coordination.
