Beginning in January 2025, the Discharge Function Score will have a sizeable impact on a home health agency’s Medicare Value Based Purchasing Score. Ambulation, bed mobility, sit-to-stand, and toilet transfers are among the functional GG items Home Health PTs will be expected to advance from SOC to Discharge. However, many of our home health patients are elderly, frail, and vulnerable and have multiple comorbidities.
