New CDC regulations: The dog MUST:

• Appear healthy upon arrival.
• Be at least six months of age.
• Be microchipped; and
• Be accompanied by a CDC Dog Import Form online submission RECEIPT> This means that for dogs (service dogs too) that have been ONLY in dog rabies-free or low-risk countries (see links below) for the last 6 months, WILL NEED TO SUBMIT THIS ONE FORM > CDC Dog Import Form <  Click COMPLETE AND SUBMIT.
