Evidence has demonstrated that effective care transitions from the hospital to home have a major impact on patient outcomes. This month, we are reviewing best practices for pharmacists practicing on both sides of the transition from hospital to home. In episode 1, APhA Member Marissa Cavaretta will discuss evidence-based strategies and real-world experiences that elevate a hospital pharmacist’s impact at discharge, including medication reconciliation, discharge counseling, and post-discharge follow-up calls aimed at preventing avoidable readmissions.
