Also note how often you go to the bathroom: the goal you should aim for is creating a bladder schedule where you pee every 2-3 hours. If you’re nowhere close to that 2-3 hours, don’t panic and don’t force yourself to hold it. A pelvic floor physical therapist can plan out a treatment path to get you where you’re at to where you want to be. **Pelvic Floor Contractions** Pelvic floor contractions are a coordinated effort with key three areas: simultaneously pulling in and lifting back of the pelvic floor, lifting the testicles upwards, and lifting the rectum towards your belly button. If you’re struggling with these movements, you’re not alone. Many men feel more confident with contraction exercises after working with a physical therapist. Chronic pain, muscle tightness, and fatigue are common complaints for those with Ehlers–Danlos syndrome (EDS); an inherited condition that can take over 10 years to diagnose. In this episode, Libby shares why the specialty model of medical care leaves many health concerns undiagnosed. Multi-system conditions, like Ehlers–Danlos syndrome, are hard to diagnose not only because many physicians are not educated about them, but also because people with Ehlers–Danlos syndrome experience vague clusters of symptoms that don’t fit within a specific medical specialty, leaving the majority of concerns undiagnosed or left with random and overlapping diagnoses. This is important to recognize when considering concerns in your pants, because the pelvic region and urogenital system are made up of collagen-rich tissues (like the bladder and pelvic ligaments). With EDS, wherever there’s connective tissue, instead of having taught tissues, the tissue sags and creates an opportunity for pelvic floor prolapse, urinary and fecal incontinence, anal fissures, vesicoureteral reflux, recurrent urinary tract infections, cryptorchidism, hypogonadism, testicular torsion, and tight foreskin. Because muscle tightness in Ehlers-Danlos syndrome is a compensatory mechanism for a lack in joint stability and muscle strength, treatment plans start with simultaneously unwinding tension while building a foundation of functional strength. Before training and strengthening muscles, it’s important to allow the muscles to relax first, since an already activated and contracted muscle can’t contract anymore. How and why to seek a diagnosis for Ehlers-Danlos syndrome: On average, it takes 10 years, leaving many suffering for a prolonged time. There’s a wide spectrum of symptom severity for people with Ehlers-Danlos syndrome. For some, the symptoms magnify quickly after surgery, viral infection, or changes in hormones (like during or after pregnancy). There are 14 types of Ehlers-Danlos syndrome, with Hypermobility Ehlers-Danlos syndrome (hEDS) being the most common. 13 of the types are diagnosable with a blood test, while the hypermobile type is diagnosed with a clinical evaluation. For healthcare providers and practitioners, rare conditions like Ehlers-Danlos syndrome are a reminder to check assumptions when considering a default diagnosis and to think outside your patterns of diagnosing by really considering all the symptoms in front of you. Electrical nerve stimulation via the skin stimulates sensory nerves that can help calm an extra sensitive bladder. When used as part of a treatment plan, Fiona saw how using TENS for bladder concerns was very effective. How fast will you know if TENS working for you? For an overactive bladder, you’ll typically see results in a few weeks (episodes aren’t as intense, more time between episodes…) and should use a TENS unit for at least 12 weeks. For an overactive bladder: Fiona recommends placing the TENS pads on the ankle over the tibial nerve. You’ll know you’re on the nerve if you turn up the pulse width and see your toes start to wiggle. Start with 10 Hz and 200 pulse width, increasing the pulse width until you start to notice the sensations. Use for about 30-minutes daily. For bladder pain: Fiona recommends placing the TENS pads over the tibial nerve or sacrum. You can either start with a low frequency 1-4 Hz or a high frequency 80-150 Hz. Use for about 30-minutes daily. When purchasing a TENS unit, make sure you can adjust the frequency (measured in Hertz), pulse rate (pulses you feel in one second), and pulse width (the amount of stimulation you feel, you want to be able to change the pulse width in small increments) to give you the most control and personalization over the unit. There are many, many challenges with pelvic pain that don’t happen if you sprain an ankle or have a shoulder injury that you can point to and talk about with your co-workers. With pelvic pain, it may feel hard to show others where it hurts— even a health professional, awkward to talk about it with friends or someone you love, that your morals or faith will be questioned because of religious beliefs, or that the pain will never stop. Dr. Sandy Hilton is a pain and pelvic health physical therapist currently in Chicago at Entropy Physiotherapy. Sandy works in the clinic and online to help people with pain recover and is an international invited speaker and instructor of other health professionals. Sandy teaches classes on manual therapy and sensory integration techniques that are purposefully non-painful and help recover tolerance to sitting, bike riding, sexual function and more!
